Based on your Internet IP address, one can quickly resolve where your Internet connection comes from as a geographical location (we use GeoIP).
With even more resources (and generally access to your ISP's data) it is often possible to narrow it down to individual streets or even your actual residence,
but very few organizations can actually do this legally.
However this can be fooled by using proxies to bounce off other remote servers, which can also be tracked down but not in all cases and which requires much greater legal expense. Even assuming that they could get sufficiently close to the proxy, real time operation is near impossible without a huge hardware infrastructure that only state intelligence agencies can afford.
